We are a dedicated provider of Home and Community-Based Services (HCBS). We believe that financial constraints should not be a barrier to high-quality care. SuBon Home Care is proud to participate in Virginia’s Medicaid Waiver programs, providing an essential alternative to institutionalization for those enrolled in:
- CCC Plus Waiver (Commonwealth Coordinated Care Plus)
- DD Waivers (Developmental Disabilities), including Family and Individual Supports (FIS) and Community Living (CL) waivers.
These programs allow our clients to control their personal resources and engage in community life to the same degree as those not receiving Medicaid assistance.
